Longfield Hospice is an award-winning, community-based service providing palliative and end-of-life care to adults and their families in Gloucestershire. Its Hospice at Home service enables people to spend their final days in familiar surroundings, with comfort, dignity and professional support.
Funding awarded in 2023 supported the salary of a specialist nurse, a key role central to delivering rapid assessments, coordinating care, and offering emotional and practical support to both patients and families, often from the point of referral through to end of life.
The impact of this funding has been substantial. During 2025/26, 279 patients received end-of-life care at home, supported by over 13,600 hours of face-to-face care. On average, the team supports around 31 patients each month, delivering hundreds of visits and ensuring patients’ needs are regularly assessed and responded to. Families are guided and reassured throughout, helping them feel supported at an incredibly difficult time.
