Nightingale House Hospice provides specialist palliative care for people with life limiting illnesses. 

The hospice catchment covers a wide region in North Wales, including Wrexham, Flintshire and East Denbighshire. Services are free of charge to patients, their families and carers, and includes bereavement support for children and young people.

A multi-year grant is currently supporting the salary of a Patient and Clinical Flow Lead, a vital front facing-role that connects patients and families with the right care at the right time. The role works closely with healthcare providers, coordinating referrals, managing admissions and discharges, and ensuring patients and families understand and can access hospice services.

The funding has improved both access to services and the coordination of care. In the last year, referrals to Nightingale House Hospice have risen by 19 percent and these are coming from a more diverse range of sources, from healthcare professionals to carers and direct requests. This means more patients receive timely joined up care, advice and support while reducing the stress of navigating complex systems.

Hospice patient
